The issue opens with a detailed case report titled “First Time in Vadodara: Fenestrated Endovascular Aortic Aneurysm Repair (FEVAR).” The case involves a 67-year-old high-risk patient diagnosed with an aortic aneurysm under surveillance for over three years. With progressive enlargement and elevated risk of rupture, conventional open surgery posed significant risk due to prior cardiopulmonary issues and altered renal function. Standard EVAR was also not feasible as a covered graft would compromise renal and mesenteric arteries. This clinical scenario necessitated a highly specialised fenestrated approach.
The procedure section outlines the meticulous planning undertaken by the vascular and endovascular team at Aadicura Superspeciality Hospital. The on-table fenestration technique involved precise computerised measurements and creation of customised fenestrations to preserve flow to both renal arteries and the mesenteric artery. Executed via a percutaneous approach using closure devices in the groin, the intervention avoided major incisions. Post-procedure imaging confirmed complete sealing of the aneurysm while maintaining perfusion to vital abdominal organs. The patient’s recovery was uneventful, and discharge occurred within three days, reflecting the efficacy of minimally invasive vascular reconstruction.
