The success story titled “Vascular Scalp Neurofibroma – A Surgical Nightmare” presents a rare and challenging neurosurgical case. A 46-year-old female with a 15-year history of a progressively enlarging pedunculated scalp mass presented with neurological symptoms and cosmetic disfigurement. MRI revealed a hypervascular lesion with multiple arterial feeders. Following unsuccessful embolisation attempts elsewhere, the team at Aadicura performed targeted endovascular embolisation using n-butyl cyanoacrylate (NBCA), achieving significant reduction in lesion size and vascularity. This was followed by complete surgical excision and reconstruction with a split-thickness skin graft.
Postoperatively, the patient experienced satisfactory healing and was discharged within one week. Histopathology confirmed neurofibroma without evidence of malignancy. The multidisciplinary team credited for this complex intervention includes Dr Sandip Mavani, Dr Ashish Desai, and Dr Vithal Rangarajan from the Department of Neurosurgery. The “Take-Home Message” emphasises the importance of a multimodal strategy integrating endovascular neurosurgeons, vascular surgeons, and plastic surgeons in managing highly vascular scalp tumours to ensure safe and effective outcomes.
