The success story titled “Right Lower Limb DVT Extending Till Right Atrium” presents a critical vascular emergency involving a 33-year-old male with progressive right leg swelling and back pain. Initial suspicion of deep vein thrombosis was confirmed through CT venography, revealing an extensive thrombus extending from the right popliteal vein through the iliac veins and inferior vena cava into the right atrium. Recognising the life-threatening risk, the Vascular & Endovascular team performed emergency catheter-directed thrombolysis. Access was secured via the popliteal vein, and thrombolytic infusion was initiated under ICU monitoring. Over 24 hours, significant clot resolution was achieved, restoring venous flow and preventing catastrophic complications. The patient showed steady recovery with reduction in swelling and improved mobility. The case underscores the aggressive nature of ilio-caval thrombosis with atrial extension and highlights the importance of rapid recognition and timely endovascular intervention. The procedure was led by Dr Vijay Thakore, Dr Sumit Kapadia, Dr Hiten Patel, Dr Kushan Nanavati, and Dr Parth Joshi.
