The issue opens with a detailed report titled “Bilateral FD Placement in a Case of Mirror Blister Ophthalmic ICA Aneurysms.” The case involves a 50-year-old male with a history of hypertension who presented with sudden severe headache and double vision. Imaging revealed left-sided Sylvian and basal subarachnoid haemorrhage, and subsequent Digital Subtraction Angiography with 3D rotational angiography confirmed bilateral ophthalmic internal carotid artery blister aneurysms. The ruptured left-sided aneurysm was treated with Flow Diverter stent placement, followed by staged treatment of the contralateral aneurysm during the same admission. The case underscores the importance of advanced endovascular techniques in managing high-risk cerebrovascular pathologies.
The “Take Home Message” section further emphasises that spontaneous subarachnoid haemorrhage must be thoroughly investigated using DSA and 3D rotational angiography, even when conventional CT angiography appears normal. Blister aneurysms, often challenging to detect and treat surgically, can be successfully managed with vessel reconstruction strategies using modern hardware and specialised infrastructure.
